  • Parkinson’s disease
  • Post-traumatic stress disorder
  • Sickle cell anemia
  • Severe chronic or intractable pain
  • Terminal illness (defined as 12 months or fewer to live)
  • Tourette syndrome

Physician Certification

Doctors, physician’s assistants, and advanced practice nurses can authorize patients for medical marijuana and ARE NOT required to have a bona fide provider-patient relationship with the patient.
